I hereby agree to participate in Marin County Tennis Camp and Clinics upon the understanding and condition that:

I recognize the risks of illness and injury inherent in any exercise program and am participating upon the express agreement and understanding that I am hereby waiving and releasing Marin County Tennis Camp and Clinics from and against any and all claims, costs, liabilities, expenses and/or judgements, including attorney’s fees and court costs arising out of my participating in Marin County Tennis Camp and Clinics. This does not include any illness or injury directly resulting from gross negligence or willful misconduct on the part of Marin County Tennis Camp and Clinics. All students must be covered by medical insurance to attend camp or clinic.

I give permission to have the following treated in the case of an accident or illness.

I give permission for Marin County Tennis Camp and Clinic to use any photos taken at camp or clinics for advertising purposes.

I hereby execute and deliver this Waiver and Release to induce Marin County Tennis Camp and Clinics to permit me to participate in its camp or clinic. In Witness Hereof, I have executed this Waiver.
